Ayushman Card Eligibility: Who Can Apply Online in 2026?
Ayushman card Eligibility is based on the Socio-Economic Caste Census 2011 (SECC 2011). The scheme has two separate categories: rural households and urban households. In March 2026, the government made two major expansions. All senior citizens aged 70 and above are now covered, regardless of income. ASHA workers, Anganwadi workers, and their families were also added. These changes brought in an additional 6+ crore beneficiaries.
Rural Household Eligibility
Any rural family meeting at least one of the following conditions qualifies:
- Family living in a single-room home with unplastered (kachha) walls and roof
- No adult male member between the ages of 16 and 59
- Female-headed household with no adult male
- A disabled member with no other able-bodied adult earner
- Scheduled Caste or Scheduled Tribe family
- Landless households whose main income comes from manual casual labour
- Primitive tribal groups, bonded labour, or families surviving on begging
Urban Household Eligibility
Urban families qualify if the primary earner works in one of these jobs:
- Domestic workers, street vendors, rag pickers, cobbler/tinsmith/hawker
- Construction workers — mason, plumber, painter, welder, labourer
- Transport workers — driver, conductor, helper, rickshaw puller, cart puller
- Shop assistants, peons, delivery assistants, and waiters in small establishments
- Security guard, sweeper, sanitation worker, head-load worker
- Home-based workers, artisans, or handicraft workers
ASHA and Anganwadi Workers (2026 Inclusion)
37 lakh ASHA workers, Anganwadi workers, Anganwadi helpers, and their immediate families were added to PM-JAY in 2025. If you work in one of these roles, you now qualify even if your household does not appear in the SECC 2011 data.
Who Does Not Qualify?
The following households are excluded from PM-JAY:
- Families with a registered motorised four-wheel vehicle (cars, tractors)
- Income tax filers
- Government employees drawing a regular salary
- Families owning more than 2.5 acres of irrigated agricultural land
- Families in a pucca house with three or more rooms
- Households with a refrigerator and a landline connection
Documents Required for Ayushman Card Apply Online 2026
Keep these documents ready before you start the application. Missing even one can cause delays.
- Aadhaar Card: Mandatory for identity verification and OTP-based eKYC. Your mobile number must be linked to Aadhaar to receive the OTP.
- Ration Card: Used to verify family membership. Helps confirm which household members are part of the same family unit.
- Registered Mobile Number: Must be the same number linked with your Aadhaar. All OTPs during the online process are sent to this number.
- Voter ID or Driving Licence: Accepted as secondary identity proof if needed during offline verification at a CSC.
- Caste Certificate: Required for SC/ST families applying under the rural deprivation category.
- SECC 2011 Beneficiary Confirmation: Your name or the family head’s name must appear in the PMJAY beneficiary portal database. You can check this at pmjay.gov.in.
Note: If your mobile number is not linked to Aadhaar, you cannot complete eKYC online. Visit your nearest Aadhaar Seva Kendra first, update the mobile number, then return to the application.
How to apply online for the Ayushman Card in 2026?
There are three ways to apply: through the official website, through the Ayushman App, or offline at a Common Service Centre (CSC). The online process is the fastest.
Apply via the official website
- Open beneficiary.nha.gov.in on your mobile or computer browser.
- On the homepage, click the PMJAY Beneficiary portal from the login options.
- Enter the OTP received on your mobile. You are now logged in to the beneficiary dashboard.
- Select your State, Scheme, and District from the dropdown menus.
- If your name appears, your family is listed as eligible. Select the family member for whom you want to create the card.
- Click the eKYC icon next to that member’s name.
- Choose Aadhaar OTP as the authentication mode. Enter your Aadhaar number, tick the consent box, and click Allow.
- Enter the OTP sent to your Aadhaar-linked mobile. Your identity is now verified.
- Wait 15 to 20 minutes after eKYC. Then click Download Card.
- Your Ayushman Card will download as a PDF. It contains your PM-JAY ID and QR code. Print it or save it on your phone.
Apply via Ayushman App
The official Ayushman App is available on the Google Play Store and Apple App Store. It is published by the National Health Authority (NHA), Government of India. The app also supports the Kashmiri and Urdu languages in addition to major Indian regional languages.
- Download and open the Ayushman App.
- Log in with your mobile number and OTP
- Complete eKYC using Aadhaar OTP
- Download your card directly from the app
Offline at Common Service Centre (CSC)
If you cannot complete the process online, visit the nearest Common Service Centre. This is especially useful for senior citizens or those without Aadhaar-linked mobile numbers.
- Find your nearest CSC at locator.csccloud.in
- Carry your Aadhaar card, ration card, and your registered mobile number
- The Village Level Entrepreneur (VLE) at the CSC will complete the application on your behalf
- The service is completely free. Report to 14555 if anyone asks for payment.
How to Check Your Name in the Ayushman Card Beneficiary List 2026?
Before you apply, confirm that your family is listed in the PM-JAY beneficiary database. This takes under two minutes.
- Go to pmjay.gov.in and click Am I Eligible?
- Enter your mobile number and the OTP received.
- Search by your name, ration card number, or Aadhaar number.
- If your name appears, you are eligible. Proceed with the application at beneficiary.nha.gov.in.
- If your name does not appear, visit your nearest CSC, the Ayushman Mitra desk, or the Ayushman card status check at any empanelled hospital. They can check if you qualify under an expanded category or help with a grievance.
Get Your Card After Approval
Already completed eKYC? Follow these steps to download your Ayushman Card anytime. Your Ayushman Card does not expire. You can re-download it anytime by logging in again. The Rs 5 lakh annual benefit resets every financial year, but the card itself stays valid for a lifetime.
- Log in to beneficiary.nha.gov.in with your mobile OTP.
- Go to your family member list on the dashboard.
- Select the member whose card status shows Approved.
- Click Download Card.
- Complete Aadhaar OTP authentication to confirm your identity.
- The card downloads as a PDF file containing your PM-JAY ID and QR code.
- Print and laminate the card for hospital use, or keep the digital copy on your phone.
How to Apply Online for Ayushman Card State-Wise in 2026?
The national portal beneficiary.nha.gov.in works in every state. Some states also run their own portals or expanded schemes alongside PM-JAY.
Gujarat
Gujarat has a dedicated state portal at ayushmanbharat.gujarat.gov.in. Government employees and pensioners in Gujarat apply through a separate PMJAY-G category form. Both the national and state portals accept applications.
Delhi
Delhi has its own health scheme for residents. Migrants from other states can use their home state eligibility on the national portal. Check both the national portal and the Delhi government health portal to cover all options.
Bihar
Bihar has a large beneficiary base. Ayushman Melas’ dedicated enrollment camps are held regularly across districts. In 2025, 1.5 crore additional beneficiaries were added in Bihar through these camps.
Uttar Pradesh
UP has the largest number of PM-JAY beneficiaries in India. Dedicated Ayushman Bharat Melas are conducted at the block level throughout the year. Apply at the national portal or attend the nearest mela for on-site assistance.
Rajasthan
Rajasthan runs the Chiranjeevi Yojana alongside PM-JAY. Both schemes are linked. Check your eligibility on the state portal at chiranjeevi.rajasthan.gov.in as well as the national PM-JAY portal. Eligible families may benefit from both.
Common Problems While Applying Online and How to Fix Them
Most application failures come down to a few fixable issues. Here is what actually goes wrong and what to do.
Name Not Found in Beneficiary List
Check spelling variations of your name. Try searching by Aadhaar number instead of name. If still not found, visit a CSC with your ration card. Call 14555 and ask for a manual beneficiary check. If you are a senior citizen aged 70+, you qualify; go directly to the 70+ enrollment link on the portal.
OTP Not Received During eKYC
Your Aadhaar-linked mobile number may be inactive or changed. Visit your nearest Aadhaar Seva Kendra or book an appointment at uidai.gov.in to update the mobile number. Once updated, return to the application process.
eKYC Failed or Biometric Not Matching
Senior citizens and manual labourers often have faded fingerprints, so biometric authentication fails. Request the Iris scan option or switch to OTP-based Aadhaar authentication instead. Both are accepted and legally valid.
Name Spelling Mismatch Between Aadhaar and PMJAY Records
Even one incorrect letter blocks eKYC. First, correct the spelling on your Aadhaar at uidai.gov.in or at your nearest enrollment centre. Once the Aadhaar is updated, the mismatch is resolved,s and you can proceed.
Card Stucinon Pending Status
Normal processing after eKYC takes 15 to 20 minutes. If the card shows pending beyond 48 hours, call 14555. Complaints are addressed within 24 hours. You can also visit the Ayushman Mitra desk at any empanelled hospital and request on-site assistance.
Family Member Missing From the Card
New family members can be added through a CSC with a ration card or birth certificate as proof. Marriage can be used to add a spouse. Visit a CSC with the supporting document, and the addition is done in minutes.
